Teachers' Gift Form
The Middleton PTO would like to offer a gift to each classroom to show our appreciation for all you do to help our children learn and grow. Simply purchase classroom supplies that you feel would benefit your students that are not available through other funding means. Each full-time teacher can be reimbursed up to $100, and each part-time teacher up to $50.
Please fill out the form below, attach your receipts (copies are fine), and leave the form in the PTO basket in the office by January 31, 2010.

Teachers' Funding Request Form
If the item(s) you have in mind exceed the gift amount we have designated for the Teacher Gift, you have the option to submit a separate Funding Request form (below). The MPTO board will review and vote on each special funding request, and notify the submitter whether or not the request is approved.
